The study on the Chemical Stability of Sodium Nitroprusside

Abstract
Sodium nitroprusside is a valuable agent in the treatment of hypertensive crisis. However, there has been a problem in administering this drug, that is, instability primarily due to photosensitivity. Currently, at Seoul National University Hospital this drug is being administered by continuous infusion via ¥³ tube protected from light with plaster wrapping or foil-wrapping. However these administration techniques have not been evaluated for the safety of sodium nitroprusside in clinical settings. Simulating the actual ¥³ administration conditions, 3 types were designed as the followings 1) administering solution with bottle and infusion set exposed to light, 2) administering solution with bottle and infusion set protected from light with foil-wrapping, and 3) administering solution with bottle protected from light with foil-wrapping and infusion set with plaster-wrapping. All procedures were performed under sun light at day time and fluorescent light at night. Aliquots of solution were sampled at time 0, 2, 4, 6, 9, 24, 48, 72, 96, 120, 144, and 168 hours after starting dropping. Each sample was tested for measuring the concentration of sodium nitroprusside by using HPLC and also UV spectrum. As the results, the concentration of sodium nitroprusside was reduced to 80£¥ at 4 hours, 50£¥ at 24 hours, and 30£¥ at 7 days after starting infusion under condition 1), no significant changes of sodium nitroprusside concentration were observed for 7 days under condition 2), the concentration of sodium nitroprusside was reduced to 98¡­99£¥at 24 hours and 85¡­90£¥ at 7 days under condition 3). Therefore, it has been concluded that sodium nitroprusside is stable for 7 days when administered with bottle and infusion set protected from light with foil-wrapping and may be stable for at least 24 hours when administered with plaster-wrapping.
